FPC Youth travel multiple times throughout the year for day trips, retreats, and conferences. All youth traveling with FPC must complete the following medical release and information form. We desire to minister to our students in the safest way possible, but emergencies do arise from time to time. These forms help us during those rare occasions to seek medical treatment or contact parents regarding medications or limitations. Thanks for your help!
